Job Description

The Targeted Case Manager (TCM) provides person-centered, trauma-informed care coordination and advocacy to clients enrolled in Arch to Freedom's Medication-Assisted Treatment (M.A.T.) Program. The TCM assists clients in identifying recovery goals, reducing barriers to treatment, and accessing needed medical, behavioral health, and community support services. This role supports clients in stabilizing their recovery, improving daily functioning, and building long-term wellness.

The ideal candidate will have experience working with individuals in recovery from Substance Use Disorder (SUD) and individuals living with Severe and Persistent Mental Illness (SMI). The ideal candidate demonstrates empathy, patience, and a strong understanding of trauma-informed and recovery-oriented care. This individual is able to work effectively within a multidisciplinary team while also being self-directed in managing time, caseload responsibilities, and documentation requirements.

Primary Duties & Responsibilities:



Conduct intake assessments, biopsychosocial evaluations, and develop individualized service plans based on client needs and strengths. Provide case management services including resource referrals, advocacy, care coordination, and ongoing progress monitoring. Support clients in maintaining engagement in M.A.T. services, including appointment coordination, pharmacy communication, and compliance monitoring. Link clients to community-based services including housing assistance, employment programs, mental health treatment, primary care, peer recovery support, and social services. Educate clients on recovery support pathways, harm reduction strategies, relapse prevention, and healthy coping skills. Maintain accurate and timely documentation in the Electronic Health Record (EHR) in compliance with agency, state, and federal standards. Participate in interdisciplinary team meetings and collaborate regularly with the medical and clinical care team. Assist clients in developing self-advocacy skills and connecting with supportive recovery-oriented activities (AA/NA, SMART Recovery, faith-based supports, etc.). Provide crisis intervention and stabilization as needed. Maintain confidentiality, professional boundaries, and HIPAA compliance at all times.
