About the Role

We are seeking dedicated

Targeted Case Managers (TCMs)

to join our team in

Miami-Dade, Broward, and West Palm Beach Counties

.
The TCM will provide

home visits

, complete

assessments

, create

service plans

, and coordinate community resources to help clients reach their goals.

Key Responsibilities

Conduct in-home and community assessments. Develop and monitor individualized service plans. Link clients to community resources and supports. Educate clients and families on available services. Maintain accurate case documentation per

HIPAA

standards. Participate in supervision and team meetings. Build caseloads through

community outreach

and referrals.

Qualifications

Bachelor's degree

(required).

Florida Certification Board (FCB)

certification as a Certified Behavioral Health Case Manager (CBHCM)(required). Knowledge of community resources in South Florida.

English and/or Spanish fluency required.

Strong communication and documentation skills.

Valid driver's license

and reliable transportation. Authorized to work in the U.S.

Job Type: Contract

Pay: $29.00 - $30.00 per hour

Expected hours: 20 per week
