What you will do:
Responsible for providing housing focused case management services to individuals and families experiencing homelessness. This position will assist homeless individuals in locating and maintaining housing and to achieve an optimum quality of life through developing plans, enhancing life skills, addressing health and mental health needs, engaging in meaningful activities and building social and community relations. Multi-county job related travel within the 11-county service area is required.
Essential Duties and Responsibilities: Other duties may be assigned

  • Provide weekly housing focused case management to households experiencing homelessness to provide support services for housing stability as outlined in their housing plan.
  • Monitor tenant\xe2\x80\x99s public housing authority, lease agreement, medical services program match support and report monthly to Program Director.
  • Document all conversations and meetings with clients and involved agencies through written case management notes.
  • Create empowered housing plans to identify needed resources to reduce the recidivism of homelessness including, identifying job placement, education/training, treatment services, financial literacy programs, mental health services and related activities, as necessary.
  • Provide advocacy and mediation with landlords to develop a workable plan to obtain or maintain participant\xe2\x80\x99s housing.
  • Provide referrals to services such as primary and specialist health care, mental health and substance abuse treatment, intensive case management, benefit
  • Locate and secure, safe and affordable housing following the regulations set forth by state and federal guidelines for income eligibility and fair market rent for assigned counties.
  • Coordinate, schedule, and conduct Housing Quality Standards and habitability inspections based on the U.S. Department of Housing and Urban Development and Michigan State Housing Development Authority standards.
  • Verify homelessness by locating individuals at their current place of living, which may include in a car, camper, tent or generally outside.
  • Prepare and maintain client files as required by NEMCSA and applicable funding sources. Remain abreast of changing guidelines in determining eligibility and record keeping.
  • Effectively utilize multiple means of technology, including computers, cell phone, tablets, Safety apps, Microsoft Office products and others as required to complete daily job-related functions.
  • Work with homeless and economically challenged households to identify potential eligibility for mainstream resource benefits and assist with appropriate referrals and follow up.
  • Maintain knowledge on program guidelines and follow them appropriately to keep in compliance with the grants.
  • Maintain accurate records for the purpose of deriving statistics, meeting legal requirements for program and client records, and compiling program reports as may be requested by program management.
  • Multi county job related travel is regularly required.
  • Attend appropriate local action committees, planning groups, departmental meetings and interagency service teams as required.

Education and Experience: Associate\xe2\x80\x99s Degree in Human Service, Business, Education, Counseling, Social Work or related field and one (1) year work experience in the human service field, proficiency in Microsoft Office Products, working with the economically disadvantaged population, as well as good communication skills and the ability to establish and maintain effective working relationships with co-workers, the community, and participants is required. Bachelor\xe2\x80\x99s degree preferred.
Certificates, Licenses, Registrations: Employees in this position must possess reliable transportation and a valid Michigan driver license. Must obtain Housing Quality Standards (HQS) Inspection Certificate and Homeless Management Information Systems Certificate. What You\xe2\x80\x99ll Love About Us:
