ENTRY LEVEL
Responsible to the Support Coordinator Supervisor for following a person-centered, enablement approach to assist people with developmental disabilities and/or related conditions in making personal choices and identifying and accessing services and supports which lead to as satisfying and normative a community life style as possible in accordance with Alabama Medicaid Agency; Alabama Department of Mental Health and applicable federal, state and local laws, requirements, guidelines and standards and Montgomery 310 policies and procedures.
Assists with people with developmental disabilities in gaining and maintaining, if eligible, Medicaid- waiver services and/or in accessing needed medical, social/ psychological, physical, residential education and other services.
Provides needs assessment, including a written comprehensive assessment of the person's strengths, weaknesses, needs, and desires.
Provides service arrangement including facilitation and participation in the development, review, and implementation of the Plan of Care( POC) within appropriate time frames. Identifies appropriate actions required to meet the defined service needs of the person and assists with implementation of those actions.
Provides service arrangement, including assisting the person in identifying necessary services and supports and in linking the person with needed services/ resources as identified in the POC.
Provides social support, including increasing the person's use of existing community support services, coordinating and/or developing services where none exist, maximizing effective use of available resources and reducing the needs for institutional care.
Provides reassessment/follow-up, including evaluating progress toward accomplishing objectives specified in the POC and the Case Management Plan at least at quarterly intervals, contacting agencies/programs providing services to the person in order to evaluate progress/effectiveness or services provides, documenting objectives/progress and participating in any revisions needed in the POC.
On an ongoing basis, monitors provided services, determines the adequacy of those services to meet the person's needs and documents needs, referrals and outcomes.
Provides reports on and completes Medicaid billable services on a schedule and at a rate established by the Montgomery 310 Case Management/ Support Coordinator Supervisor and Executive Director.
Attends and participates in interdisciplinary and interagency meetings and serves as the person's advocate and intermediary if the person is unable to do so on her/his own behalf.
Completes necessary documentation as required for maintenance of the person's service coordination/case management record as well as for accountability and billing purposes.
Prepares documents and other narrative/statistical reports to meet designated timelines as may be required by DMH or the Alabama Medicaid Agency.
Attends workshops and conferences as required by DMH and MASPMR.
Attends staff trainings, planning and work related meetings.
Performs travel in support of job requirements.
Performs related work as necessary and assigned by the Case Management Service Coordinator Supervisor.
Requires Bachelor of Arts or Bachelor of Science Degree from an accredited college or university, in a human services related field or a social work program.
Human services field includes the following disciplines: Social Work, Psychology, Criminal/Juvenile Justice, Special Education, Sociology, Speech Education, Rehabilitation, Counseling, Speech Pathology, Audiology, Physical or Occupational Therapy, and any related academic disciplines associated with the study of Human Behavior, Human Skill Development, or Basic Human Care Needs.
