Join our dedicated healthcare team as a SUD (Substance Use Disorder) Nurse Care Manager, working 4 hours per week to provide vital support for individuals navigating recovery and treatment. Responsibilities
Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine individual needs.
Develop and implement personalized discharge planning strategies that incorporate post-acute care options and outpatient services.
Collaborate with healthcare providers across inpatient, outpatient, and clinic settings to coordinate integrated treatment plans.
Manage intake processes for new patients, ensuring accurate documentation of medical records while adhering to HIPAA regulations to protect patient privacy.
Monitor and evaluate patient progress through diagnostic evaluations and ongoing case management activities.
Facilitate communication with managed care organizations to optimize utilization management and ensure appropriate resource allocation.
Provide education on medication management, behavioral health strategies, and infectious disease care relevant to substance use disorder recovery.
Assist in the development of patient service plans that address complex needs such as geriatrics, pediatrics, or behavioral health issues.
Support hospital-based teams by contributing expertise in ICU, trauma center, or emergency room settings when needed.
Engage in continuous quality improvement initiatives related to patient assessment accuracy, discharge planning efficiency, and overall care coordination.
Requirements
Valid nursing license with current registration; experience in critical care environments preferred.
Demonstrated experience working within Level II or Level I trauma centers; ICU or emergency medicine experience highly desirable.
Strong background in hospital medicine, medical-surgical units, or post-acute care settings.
Knowledge of physiology, medical terminology, and diagnostic evaluation techniques essential for comprehensive patient assessment.
Experience with inpatient and outpatient care coordination, including discharge planning and case management processes.
Familiarity with managed care protocols, utilization management practices, and medical records documentation standards.
Prior involvement in behavioral health or infectious disease care programs is advantageous.
Excellent communication skills for effective collaboration across multidisciplinary teams and with patients from diverse backgrounds.
