Position Overview
Senior Total Life Care (STLC) is seeking a detail-oriented and collaborative On-Site Specialist Liaison to coordinate all aspects of on-site specialist visits. This role ensures timely preparation, communication, and follow-up between on-site providers, referring providers, and participants. The position reports directly to the Health Information Management (HIM) Manager and plays a key role in supporting seamless care delivery within the Senior TLC program.
Key Responsibilities
Monitor Service Tracking for all on-site referrals and develop schedules for provider visits
Prepare documentation and deliver to on-site providers within required timeframes
Prepare clinic/day center space as needed prior to provider visits
Communicate upcoming appointments to participants and/or caregivers at one month, one week, and one day intervals
Obtain signed consent forms, upload to participant charts in Elation, and forward to providers
Follow standard operating procedures for scheduling on-site appointments
Document in Service Tracking
Update authorizations
Add appointments to CareHub calendar
Request transportation if required
Provide on-site providers with same-day notes; collect and upload notes to participant charts in Elation; route to Primary Care Provider (PCP) for review
Monitor Service Tracking for referrals requiring reports; extract notes from external EMR or request from providers
Upload visit notes to Elation and route to PCP within expected timeframes
Reschedule missed on-site visits as needed
Coordinate monthly on-site visits (e.g., Kintegra Dental)
Maintain and update the Senior TLC On-Site Calendar in SharePoint
Uphold Senior TLC's mission to support seniors' quality of life in the community, vision to be the preferred provider of individualized care, and values of respect, integrity, accountability, shared goals, and compassionate care
Perform other duties as assigned

Desired Qualifications
At least 1 year of experience working with the frail elderly population
1 year of related work preferred or2 years of on-the-job training in a medical office setting
Proven ability to coordinate provider schedules and manage healthcare documentation workflows
Education, Licensure, Registry and Certification
High school diploma or GED required
Certification/registration as a Medical Office Assistant (MA, RMA, CMA, CCMA), CNA, or LPN required
Health Requirements
Must be medically cleared for communicable diseases and up-to-date on immunizations
