As a Spiritual Care Counselor / Hospice Chaplain, here's a big-picture view of what you'll do:
Assess and support the spiritual and emotional needs of patients and families
Provide or coordinate spiritual services in line with each patient's beliefs and preferences
Participate in interdisciplinary group (IDG) meetings to help shape holistic plans of care
Conduct or coordinate religious services, memorials, and family celebrations as requested
Serve as a liaison and resource for community clergy and spiritual counselors
Document visits, communications, and care plans accurately and promptly
Offer education and guidance to staff and volunteers on spiritual aspects of hospice care
A few must-haves for our Spiritual Care Counselor / Hospice Chaplain:
Graduate of an accredited seminary or school of theology (MDiv required)
Completion of four (4) units of Clinical Pastoral Education (CPE) or equivalent theological training
Experience supporting individuals and families through grief, loss, and end-of-life care
Hospice experience preferred
Compassionate listener with excellent communication and collaboration skills
Valid driver's license, auto insurance, and reliable transportation
