Job Description

We are currently looking for a Full Time Specialty Clinic Case Manager/HIV-HCV to join our Specialty Clinic team located at the Chickasaw Nation Medical Center.


Summary:





The registered nurse case navigator/program coordinator has primary responsibility for day-to-day operations of the HIV-HCV cooperative agreement activities. These activities include, but are not limited to, assisting with the development and implementation of a broad screening program/process, reviewing results of HIV-HCV screenings, communicating result to providers and collaborating with providers to identify and monitor patients with HIV and HCV. They will also assist with the development and implementation of a case management process for identifying, referring and monitoring adherence to appointments and treatment, coordinating with a team of multidisciplinary providers.


Responsibilities:



Coordinates standard quality of care by collaborating with multidisciplinary providers to screen, facilitate access to care, and monitor patients with HIV-HCV Collaborates with providers and monitors patients with HIV-HCV, reviews and provides education regarding medication regimens, provides extensive counseling and motivation to help patients adhere to treatment regimens through nurse visits and phone calls Contacts patients for follow up diagnostic tests, referrals, appointments, or medical follow up, as appropriate Facilitates referral scheduling and forms completion as requested and approved by the medical staff Achieves and maintains currency in knowledge of all aspects of HIV-HCV prevention, treatment, monitoring, etc., for example, the Ending the HIV Epidemic initiative; the HIV National Strategic Plan: A Roadmap to End the Epidemic; the Viral Hepatitis National Strategic Plan; the Hepatitis C Elimination Strategy for AI/AN Communities document; and the goals, objectives and activities associated with these programs/strategies Participates in required cooperative agreement activities, e.g, regularly scheduled technical assistance calls with the funding agency; reporting and evaluation activities; budget oversight; scheduled site visits; and outreach and education activities as proscribed by the funding agency Coordinates and plans patient care in an effort to meet patient needs Provides routine patient care by utilizing and documenting the nursing process, including teaching patients and their families Provides and advocates age and culturally appropriate, holistic patient care based upon facility mission statements and as outlined in policies and procedures Practices in accordance with and adhering to regulations such as the Nurse Practice Act, CNDH policies and procedures, and regulatory agencies Recognizes and anticipates common changes in the physical, biopsychosocial and health needs of patients experiencing urgent, emergent or non-emergent conditions Participates in hospital, departmental, and OPS performance improvement activities to measure, assess and improve the quality of nursing care delivered to patients Maintains confidentiality, security, and integrity of data and information in accordance with HIPAA regulations Regularly attends 90% of staff meetings or obtains minutes and assumes responsibility for the information given Facilitates the orientation of new staff Exhibits prompt arrival to duty station as well as follows employee code book for time and attendance Facilitates patient care by generating and faxing/copying patient protected health information necessary for continuation of care to outside healthcare facilities and/or providers, ensuring that all records needed for this process are accurate and valid, while maintaining patient confidentiality, security and integrity of data and information in accordance with HIPAA and the Minimum Necessary Rule, and noting within the patient's EHR each disclosure according to CNDH documentation policy Provides first class customer service - courteous and helpful to all internal and external customer types Answers questions in a courteous and professional manner Greets and engages with a smile Determines needs, seeks positive experience and outcomes Completes all training and development requirements and participates in continuing education opportunities Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types Maintains general upkeep of work area and maintains a safe environment Maintains and promotes professionalism Punctual and consistent in meeting start time, work schedule, attendance and other assigned work time metrics Adheres to Chickasaw Nation dress code standards Maintains knowledge of and follows all laws, rules, regulations, policies and SOPs for assigned area, e.g. MICS, TICS, hospital standards, health codes, food sanitation, accounting principles, etc. Performs all other duties as assigned

Must be familiar with the Microsoft Office Suite of products Must be able to operate a computer and/or other equipment related to specific role (i.e. automated currency counters, calculator, projector, binding machine, other related office equipment)

Bachelor's of Nursing


Nonessential:


Bachelor's degree from an accredited school of nursing, or written plan to obtain within 4 years of hire

Credentials:



Essential:


Multi-State Registered Nurse Licensure within 90 days Pass general background check


Nonessential:


Basic Life Support (BLS) Unrestricted Oklahoma Registered Nurse License


Nonessential:


Must obtain BLS within one (1) month of hire, if not current
