Job Summary

The function of this position is to provide therapeutic services either in the home or other agreed upon community location to clients within the MCH Home Visiting programs that have been identified by their case manager as needing additional support of behavioral or mental health related issues. The Community Clinical Counselor will complete assessment(s) to determine the appropriate level of care. The purpose of this position is to assist clients in changing their lifestyle through the provision of individual/group counseling, education, and case management services.

Preferred Qualifications

Bilingual in Spanish and English Master's Degree preferred Requires Licensure as a Social Worker (LSW/LISW) or Licensed Professional Counselor (LPC/LPCC) Trained in Cognitive Behavioral Therapy Experience in providing mental health services, crisis intervention, and working with vulnerable populations Maternal and child health (MCH) experience, and the ability to work with people from different backgrounds Be current in adult and child CPR/AED at the time of hire or complete within 3 moths of the date of hire

Duties

Provide individualized evidence-based therapeutic in-home support services to individuals in the MCH home visiting program who are at high risk of experiencing postpartum depression or who have been identified as having other mental/behavior health issues. This may include but is not limited to safety planning, facilitating groups, individual/group therapeutic services, direct referrals to needed services, utilizing various treatment modalities, etc. Completes documentation of calls, interventions and other needed services, including assigned reports, accurately and in a timely fashion. Participation in regularly scheduled case conferences, staff meetings, trainings and any other requirements to maintain licensure. All other duties as assigned.
Job Type: Full-time

Pay: $31.61 - $35.12 per hour
