Responsibilities:

Provide ongoing case management in our Lifeset Program which is a model to address the needs of young people age 17-21 who are transitioning out of care and need individualized skill development and support to transition successfully into adulthood. Facilitate Family Support Team meetings. Develop individualized care (treatment) plans. Coordinate individualized services across all settings. Coordinate providers that work in the home, school, and community. Document support related activities and services. Work a flexible 40 hour/week schedule.

Requirements/Qualifications:

Bachelor's degree in a human behavioral science

and

2 years of experience in a related field. Or a Master's degree in a related field. Valid driver's license, vehicle insurance, reliable transportation, and a good driving record to allow approval to drive a company vehicle. Pass background check and fingerprinting.
