The Willows at Ramapo Rehabilitation and Nursing Center is seeking a compassionate
Social Worker
to support residents and families in a skilled nursing environment.Social Worker Responsibilities include:
Psychosocial assessments and resident/family support Discharge planning and coordination Participation in care plan meetings Documentation and regulatory compliance
Social Worker Qualifications:
BSW or MSW required SNF or long-term care experience preferred Strong communication and organizational skills
