We are seeking a
Social Worker
to support residents and families through person-centered, relationship-driven services that promote dignity, well-being, and a positive living experience.
Wage
: $55,000-$60,000/year
Schedule:
Full-Time, Monday-Friday 8:00am-4:00pm (option for four 10-hour shifts)
Caseload:
53 long-term care residents. Ideal for someone who thrives in a fast-paced environment, can prioritize effectively, multitask confidently, and adapt quickly when unexpected needs arise.
What You'll Do
Promote the principles of the Green House model to support a warm, home-like environment
Build strong, respectful relationships with residents, families, and interdisciplinary team members
Complete pre-admission screenings, social histories, care plans, and discharge plans
Meet with residents and families at admission and provide ongoing support throughout the stay
Assist with conflict resolution, transitions, and resident advocacy
Participate in care conferences and coordinate social services across departments
Complete required documentation including MDS, progress notes, and psychosocial assessments
Assist with investigations related to resident rights, grievances, abuse/neglect, or concerns
Coordinate mental health referrals and outside community services
Serve as liaison for Family Advisory Council meetings
Maintain confidentiality and ensure HIPAA compliance
What We're Looking For
Eligibility to work in federally funded healthcare programs
Strong communication, advocacy, and relationship-building skills
Knowledge of long-term care practices, regulations, and resident rights
Ability to collaborate within a multidisciplinary care team
Excellent documentation, organizational, and time management skills
Preferred Qualifications
Experience in long-term care or skilled nursing
Familiarity with the Green House model or similar person-centered care approaches
Experience in conflict resolution, crisis intervention, or advocacy
