, we're dedicated to providing Fabulous Every Day Care, Every Day to individuals recovering from brain and spinal cord injuries. We're looking for acompassionate and skilled Social Worker
to join our interdisciplinary care team and make a lasting impact on the lives of our clients and their families.Status:
Temporary
Schedule:
32 hours per week
Pay: Competitive, based on experience & education
What You'll Do
Support clients and families as they adjust to the rehabilitation process and extended care living. Identify and address psychosocial needs through individualized care plans. Provide counseling, crisis intervention, and education on resources and client rights. Collaborate closely with nurses, therapists, and physicians to ensure holistic, person-centered care. Assist with admissions, discharges, and community resource coordination. Maintain accurate documentation and uphold confidentiality and HIPAA standards.
What We're Looking For
Master's degree in Social Work from an accredited institution. Michigan licensure (LMSW) and Academy of Certified Social Workers (ACSW) credential. Experience in rehabilitation, long-term, or subacute care preferred. Strong communication, problem-solving, and interpersonal skills. Valid Michigan driver's license and ability to travel between facilities if needed.
