Offer crisis support, counseling, and resource referrals for patients and families Help plan safe discharges and transitions of care Coordinate with healthcare teams to keep patients supported and informed Connect patients to community resources for housing, food, transportation, and more

Job Requirements

Education:

Bachelor's (BSW) or Master's Degree (MSW, or willing to enter a Master's Degree program) from a school of social work accredited by the Council of Social Work Education REQUIRED

License/Certification:

Social Work Certification REQUIRED

At CHI Health CUMC-Bergan Mercy, our focus is patient-centered care. Our level I trauma center and academic medical center campus is designed to heal the body, mind, and spirit of every person in a more comfortable, less stressful hospital environment. Our full range of medical services includes trauma services, heart and vascular care, emergency services, surgery, maternity, cancer care, and diagnostic imaging.

Pay Range

$24.27 - $36.10 /hour
