Job Summary and Qualifications

CASE MANAGEMENT SOCIAL WORKER MSW \xe2\x80\x93 FULL-TIME DAY SHIFT

The Case Management Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated.

What you will do in this role:

• Acts as a liaison between the patient and community resources/post-acute care providers
• Creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care
• Assists the Case Manager RN in facilitating patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput

What qualifications you will need:

• Master of Social Work (MSW) required
• Licensed to practice in the state of Louisiana
• Board Certified Social Worker (BCSW), Licensed Masters Social Worker (LMSW) or License Clinical Social Worker (LCSW) required
• Certification in social work or case management preferred
• Acute care social work experience preferred
