Job Description

The

Medical Social Worker

ensures compassionate, high-quality care by implementing social work standards and upholding the Social Work Practice Act and organizational policies.

Key Responsibilities

Patient & Family Support: Assess psychosocial needs of patients and families; provide counseling, crisis intervention, and emotional support. Care Coordination: Collaborate with nurses, physicians, and the interdisciplinary team to develop personalized care plans. Cultural Sensitivity: Identify cultural needs and educate the care team on respectful, inclusive practices. Resource & Education: Connect patients and families to community, financial, and legal resources; assist with advance directives and end-of-life planning. Documentation & Compliance: Maintain accurate, timely clinical records per professional and organizational standards. Collaboration & Advocacy: Serve as liaison between families, caregivers, and community agencies to ensure seamless, patient-centered support.
