About the Role
The Hospice Social Worker supports patients, caregivers, and families by addressing the emotional and psychosocial needs that arise with advanced illness and end-of-life care. This role works closely with the interdisciplinary team to develop and support an individualized plan of care. The Social Worker provides counseling, assessments, interventions, and resource coordination to help families navigate challenges and maintain safety and well-being in the home setting.
How You'll Make a Difference
Assessment of Psychosocial Status
Evaluate the psychosocial needs of patients and caregivers in the context of terminal illness.
Share findings with the Registered Nurse and interdisciplinary team to guide the plan of care.
Safety and Environment Assessment
Assess safety and environmental conditions in the residence.
Recommend alternative plans if the home environment presents risks.
Social Evaluations and Interventions
Complete social evaluations to understand needs and barriers.
Develop and implement interventions that support stability and coping.
Counseling
Provide supportive counseling to patients and families related to stress, illness progression, and end-of-life issues.
Clinical Records Management
Maintain accurate clinical records for all assigned patients.
Document care and assessments directly in the EMR at the bedside.
Information and Referral Services
Offer guidance on community resources, practical supports, and environmental needs.
Connect patients and families with agencies and services that enhance care.
Liaison and Collaboration
Serve as a liaison between families, hospice team members, and community partners.
Maintain strong working relationships with internal staff to support coordinated care.
Networking and Resource Development
Build and maintain relationships with public and private agencies.
Support the development of new resources that benefit patients and families.
Interdisciplinary Team Participation
Participate in IDT meetings and help develop individualized care plans.
Educate team members on social and emotional factors that influence health and end-of-life decisions.
Quality Improvement
Participate in quality assessment and performance improvement activities.
What You'll Bring
Bachelor of Science in Social Work, Psychology, or Sociology; MSW preferred.
At least one year of experience in a health-care setting; hospice experience preferred.
Understanding of hospice philosophy and issues related to death and dying.
Strong communication, documentation, and organizational skills.
Ability to travel throughout the Blue Ridge Care service area in Loudoun County.
