About the Position:

The Social Worker SWIII, for the Mobile Health Clinic within DHHS\' Public Health Services, will interview-assess-recommend-decide-act on clients experiencing complex somatic illnesses, mental health illnesses, substance abuse issues, emotional disturbances, or social issues. The SW III will work with individuals and often families being served by the mobile health and human services unit on prevention, stabilization, recovery or enhanced functioning or well-being, or full self-sufficiency, to help them attain better social, emotional, physical or mental health.

Duties and Responsibilities:

  • Conducts holistic clinical assessments, formulates a diagnostic impression, and identifies socioeconomic, emotional, health and other problems to be solved.
  • Develops and recommends person-centered treatment plans and interventions to address identified client problems.
  • Conducts individual or family intervention, short- or long-range therapeutic counseling or other support services.
  • Provides psychotherapy for mental and emotional disorders.
  • Provides crisis intervention as necessary.
  • Provides advanced and specialized person-centered, holistic clinical case management and care coordination to clients and families of clients.
  • Conducts follow-up and advocates to resolve barriers and ensures clients are connected with medical, psychiatric, psychosocial and other essential services to meet their needs.
  • Attends, or leads, case consultation staff meetings, cross-functional, cross-unit service integration meetings and conferences; makes recommendations regarding treatment plans.
  • Leads, coordinates, or contributes to the achievement of established program goals and objectives through data analysis, policy and procedure development to ensure quality standards and safety standards.
  • Completes reports, maintains statistical data, and uses data visualization concerning clients served/services provided.
  • Prepares and maintains patient records involving progress and contact notes, internal and external correspondence in the clinical software system and in the electronic case management system.
  • Provides social and human services work leadership to the Mobile Health Clinic team.
This Social Worker will need to operate a motor vehicle to drive to locations throughout the County to perform duties and transport supplies. The is a community-based role and will require weekday evening hours as well as weekend day and evening hours.

Bilingual applicants in the Spanish/English languages are encouraged to apply. If the candidate selected for this position possesses multilingual skills, they will be required to successfully pass an examination assessing oral communication, reading comprehension, and interpreting skills. For Advanced proficiency, the candidate must also pass an examination requiring translating text from Spanish to English and vice versa.

Minimum Qualifications

Education: Possession of a Master\'s Degree in Social Work (MSW) from an accredited college or university that meets the criteria of graduate social work program accredited by the Council on Social Work Education.

Experience: Two (2) years, to include a minimum 3,000 hours, of professional experience rendering social work services under the supervision of a LCSW-C in accordance with Maryland Board requirements.

Equivalency: See Maryland Board requirements.

License:
  • Must be licensed as a Licensed Certified Social Worker - Clinical - LCSW-C by the Maryland State Board of Social Work Examiners prior to appointment.
Note: There will be no substitutions for this requirement.
  • Possession and maintenance at all times of a valid Class C (or equivalent) driver\'s license from the applicant\'s state of residence when required for job-related duties.
Preferred Criteria

There is no Preferred Criteria. All applicants will be reviewed by OHR for minimum qualifications. Those applicants who meet minimum qualifications will be rated Qualified, placed on the Eligible List, and may be considered for an interview.

Preference for interviews will be given to applicants with experience in the following:
  • Experience conducting social work practice in a public health or health care environment.
  • Ability to integrate models of person-centered care, social determinants of health, and healthy equity into clinical care practices.
  • Experience conducting social work practice to individuals and families throughout the life span (people of different ages, health conditions and disabilities).
  • Knowledge and skill in developing, revising, and maintaining programs, policies and procedures or developing new programs, policies and procedures and helping to implement them.
