Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 24 months of Social Work experience within the past 3 years.
Trauma Level 1 experience is preferred from any number of months
Experience with Epic is required from any number of months.
License
Candidates must have a license

Additional Details:

You must be able to complete labs within 48 hours of signing.

Required Skills/Experience:

Skills: Outpatient clinic, assessments and evaluations, establish and revise goals, plan of care, collaborative care, documentation, adult and child protective services, psychiatric disorders, Billable codes under DSM-5-TR, Individual, couples, parent-child and family therapy sessions. Potential to facilitate IOP group sessions. Will need to have an IL clinical license, NPI number and be enrolled with Medicare and Medicaid so they can bill both hospital and professional fees for their services, Knowledge of Protective services, Adult protective services, Child protective services, Assessment, Assess response to treatment, Biopsychosocial assessment, Conduct initial evaluation, Develop plan of care to meet patient/caregiver needs, Establish and revise goals, Care Coordination, Collaborative care, Coordinates with internal and external team members, Refers to other disciplines/resources, Documentation/Communication, Discharge Summary, Documentation according to plan of care, Documentation of care delivery, plan, goals, etc, Documentation of patient/caregiver teaching/learning, Documentation reflects understanding of coverage criteria, Documents patient response, Counseling/Therapy, Family therapy, Group Therapy, Individual counseling, Professional Knowledge and Skills, High risk intervention patient vulnerability or safety risk, Knowledge of cultural issues, Work Settings, Outpatient Clinic, Knowledge of Community programs (income/insurance), Knowledge of Major Disease Processes, Psychiatric Disorders, Knowledge of specialized areas, Suicide/homicide/self-harm

Preferred Skills/Experience:

Skills: Guardianship, Power of Attorney, Development of problem list, Documents pertinent information to facilitate care coordination, Facilitate discharge and contingency planning, Marriage counselling, Parental counseling, Provide crisis intervention, Patients with communication problems (hearing, speech), Prior Authorization, Provide / reinforce individualized patient/caregiver education.

Required Credentials:

Certifications: Must have of the three: LCSW, LCPC or LMFT. ID: Drivers License/State ID, Government-Issued Photo ID.

Shift & Scheduling:

No weekends or holidays. Monday-Friday.
