**This position may be eligible for a $3,000 sign on bonus with a 2-year commitment.**

- Work with a variety of patient concerns.
The social worker is responsible for planning, organize and implementing social work services for patients and families, assuring that the medically related emotional and social needs are addressed on an individual basis. Responsible to assess discharge needs and facilitate with patient/family. Plan for discharge and aftercare. Maintain the current knowledge base of community agencies and resources to facilitate appropriate referrals. The social worker provides services in a compassionate, culturally competent and tactful manner in order to help facilitate access to and provide continuity of care and patient advocacy. Work collaboratively with other members of the multidisciplinary team.
Education and/or Experience
- A Bachelor\'s degree in social work from a four-year college or university is required.
- Licensed as a Social Worker in the State of South Dakota required.
- Varied Shifts; 48hrs/2wks
The hourly range for this position is listed below. The actual hourly rate is dependent upon experience.
- $21.50 - $32.50 per hour
