You will assist veterans and their family members through admissions and financial affairs, as well as assisting the Social Service staff.
Minimum Qualifications

Possession of a current Louisiana license to practice as a Licensed Master's Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW) plus:
A master's degree in social work followed by one year of social services experience.

Job Duties and Other Information

Job Duties:
Serves as Case Manager for medically complex, intermediate and skilled care including Medicare veteran units with a caseload of veterans with multiple psychiatric, substance abuse, behavioral and medical problems. Resident care in a 156-bed long-term care facility.
May escort and assist veterans to medical and other appointments.
Responsible for admissions protocol involving financial affairs, medical records and placement on their assigned units and assist other Social Service staff.
Investigates and reports resident complaints and incidents, recommending corrective actions and collaborating with Supervisor.
Updating veterans face sheet, represents department ay monthly Residents meetings.
