A Brief Overview





The Manager of Social Work works with the Directors of Hospice Nursing and Social Work and other collaborative directors and managers to coordinate the implementation, delivery, and supervision of the discipline of social work ensuring services are consistent with the mission, vision, and values of Holy Name. The Manager of Social Work works primarily with supportive care departments including hospice, home care and palliative care, leading by example in their own social work practice and is responsible for meeting all regulatory standards and quality outcomes as defined by the organization, state, and federal guidelines for the departments.



What you will do



Manages the clinical operations of social workers in hospice and home care. Supports the Director of Social Work in the oversight of other departments offering social services (e.g. palliative care, continuous care, etc.) Works with Hospice Interdisciplinary Team to ensure all patient and family spiritual and psychosocial needs are met. Collaborates with social workers across departments to assist with complex cases. Meets with employees as needed to review opportunities or unique issues from workload, shares professional and educational support, and exchanges feedback aimed towards enhancing professional growth. Provides clinical supervision as administrator on call and for social work employees to advance clinical licensure. Able to coach, mentor and develop staff. Demonstrates skill in encouraging and achieving employee satisfaction as evidenced by retention of qualified and competent staff. Implements process improvement initiatives with measurable outcomes to meet departmental and organizational goals. Fosters teamwork by initiating and building communication with all levels of management and employees to promote cohesiveness and improved outcomes. Participates in the development, revision, and implementation of policies and procedures, goals, and objectives; ensures standards of care relevant to the discipline of social work. Oversees maintenance of medical records to ensure accuracy, completeness, and compliance with licensing, regulations, accreditation, and legal/ethical standards for social work services; ensures staff documentation is per standards. Conducts supervisory visits to evaluate performance of employees as needed or at least annually; provides employee evaluation per Holy Name standards as assigned. Performs and documents comprehensive psychosocial and pre- bereavement assessments and assists in the development and implementation of the plans of care according to Holy Name standards; attends IDT as needed. Assists other Social Workers, Spiritual Counselors and Bereavement Counselors with their patient/family assignments as needs are identified.

Education Qualifications



Master's Degree in Social Work Required

Experience Qualifications



5 years of experience in a healthcare setting Required 5-7 years experience in clinical social work, including counseling patients and/or family members Required

Knowledge, Skills, and Abilities



Strong assessment and documentation skills for supervision, organization, and documentation Strong interdisciplinary communication and collaboration skills

Licenses and Certifications



LCSW- License Clinical Social Worker Valid New Jersey LCSW License Upon Hire Required DL NUMBER - Driver License, Valid and in State Valid New Jersey Driver's License Upon Hire Required

Pay Range: $119,995.20 - $143,998.40
