Position Overview


We are seeking an experienced, motivated, and empathetic

Social Worker Director

to lead our Social Services Department. The ideal candidate will be a licensed social worker with a strong background in sub-acute demonstrated leadership abilities, and a passion for advocating for patients and families.

Key Responsibilities



Lead, supervise, and support the Social Services team to ensure compliance with all federal, state, and facility regulations. Conduct comprehensive psychosocial assessments for new admissions. Develop and implement individualized care plans in collaboration with interdisciplinary team members. Support patients and families through discharge planning, including care coordination, community resources, and follow-up services. Address resident concerns, grievances, and emotional or psychosocial issues. Serve as liaison between residents, families, medical providers, and outside agencies. Maintain accurate, timely documentation within EMR systems and according to regulatory standards. Provide staff training and ongoing professional development to enhance the quality of social services. Participate in care plan meetings, utilization reviews, and facility committees as needed. Foster a caring, inclusive environment where residents feel supported and empowered.

Qualifications



Active and valid Social Worker License (LSW or LCSW) required.

Bachelor's or Master's degree in Social Work (MSW preferred). Minimum 2 years of experience in a healthcare, sub-acute setting. Prior leadership or supervisory experience strongly preferred. Strong communication, organizational, and critical-thinking skills. Ability to work collaboratively across interdisciplinary teams. Knowledge of discharge planning, community resources, and healthcare regulations.
