Job Overview
We are seeking licensed social workers (LSW, LCSW, LICSW) to join our team. The ideal candidate will possess a strong background in medical social work and a passion for supporting clients and residents. Our social workers play a crucial role in supporting our clients through various challenges such as temporary staffing vacancies, significant administrative changes and DPH survey preparation.

Responsibilities vary but often include:
Provide supervision and support to facility and agency social workers
Collaborate with facility and/or agency IDT to improve efficiency and effectiveness of current procedures and practices while ensuring compliance
Conduct comprehensive patient assessments to determine short and long term needs and care plan accordingly.
Maintain accurate medical records and documentation in accordance with legal and ethical standards. Ensure compliance with HIPAA regulations.
Collaborate with multidisciplinary teams to coordinate care for clients and ensure effective communication
Complete ongoing documentation and adjust care/treatment plans as necessary based on ongoing assessments.
Educate clients and families about community services available, mental health issues, and coping strategies
Develop and implement plans of corrective action with facility IDT
Preferred Skills/Qualifications:
Social Work license (LSW, LCSW, LICSW) strongly preferred
Able to effectively communicate and coordinate amongst a variety of disciplines
Experience working in a SNF and/or ADH setting, proficient in medical records management and documentation standards.
Solid understanding of social work principles and ethics.
Knowledge of HIPAA regulations to ensure client confidentiality and data security.
