Social Services Designee



POSITION SUMMARY




The Social Services Designee interacts with the residents, families, visitors, other facility staff, and community agencies during the admission and discharge process. They are responsible to market the services we provide and establish relationships with all referral sources. They clarify a range of legal documents for residents and families as well. As a member of the interdisciplinary care planning team, the Social Service position identifies the psychosocial strengths and needs of each resident, assists in developing an individualized plan of care, and provides medically-related social services to help the resident attain or maintain the highest level of functioning possible.

RESPONSIBILITIES



Coordinates Admissions and Discharges:

Manages referrals, pre-admission screenings, tours, approvals, and paperwork; ensures timely and accurate documentation and communication with relevant parties.

Supports Residents and Families:

Assists with emotional adjustment, psychosocial needs, advanced directives, financial aid applications, and room changes; promotes family involvement and provides individual support.

Collaborates on Care Planning:

Completes social assessments, mental health screenings (PHQ-9, BIMs), and MDS/CAA documentation; participates in interdisciplinary meetings and care plan development.

Maintains Accurate Records and Documentation:

Keeps thorough, up-to-date medical and departmental records per policy, including admission summaries, quarterly notes, discharge plans, and insurance/financial information.

Demonstrates Professionalism and Communication:

Maintains respectful, cooperative relationships with staff, residents, families, and visitors; adheres to dress code, communication standards, and confidentiality protocols.

Ensures Regulatory Compliance and Quality Improvement:

Follows all state and federal regulations, QAPI initiatives, and abuse prevention policies; attends training, understands residents' rights, and contributes to policy development.

QUALIFICATIONS



Must recognize and understand the psychosocial and emotional needs of residents. Experience in healthcare, skilled nursing facility or assisted living community Must have strong skills in communication, organization, management, and leadership. Must be able to perform essential functions of the position with or without reasonable accommodation. Must qualify for employment after a criminal background study, per guidelines of the Minnesota Department of Human Services.
