Key Responsibilities:

Conduct patient assessments to determine their social service needs. Develop and implement personalized care plans in coordination with an interdisciplinary team. Provide counseling and emotional support to patients and their families. Advocate for patient needs within the facility and in the broader community. Maintain detailed and confidential patient records. Oversee discharge planning and coordinate with community resources to ensure continuity of care.

Qualifications:

Master's Degree in Social Work or a related field required Current LCSW certification in the state of SC Experience in long-term care health settings, particularly in social services. Strong understanding of case management and care coordination. Exceptional communication and interpersonal skills. Ability to work compassionately with a diverse patient population.
