Job Description

The SOAR Case Manager works as a team member on site at the Youth Focus Program, My Sister Susan's House (MSSH) and will provide

services for clients

in the MSSH, HEARTH TLP and HEARTH Beyond Program by assisting with the successful integration of both the housing and individual goals of each client by meeting with housing residents to complete day to day case management, assist young adults in applying for mainstream benefits, supporting youth to develop goals towards education, employment, independent life skills and other domains to prepare for self-sufficiency. The SOAR Case Manager is responsible for assessing clients' eligibility for SOAR services, collecting, and summarizing disability documentation, and submitting final applications to access mainstream benefits on behalf of clients. This position will help ensure youth are accessing needed health insurance coverage through Medicaid and other resources, as well as other public assistance benefits according to their needs. The SOAR Case Manager must demonstrate the Agency's purpose, philosophy, policies and procedures.

Minimum of 21 years of age. Education and/or Experience - Bachelor's degree from four-year college or university; or one to two years' experience and/or training; or equivalent combination of education and experience. Language Skills - Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of clients or employees of organization. Mathematical Skills - Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Reasoning Ability - Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Certificates, Licenses, Registrations - Valid North Carolina Driver's License.



ESSENTIAL DUTIES AND RESPONSIBILITIES






Will complete all initial and ongoing training for SOAR certification Develops individual treatment plans for all residents on caseload. Collaborates with the Program Manager regarding treatment plans. Assists clients in progressing through their established treatment plans. Provides weekly case management services, focusing on education enrollment, employment, connections to community resources, health needs, recreational and other needs of housing residents. Coordinates services with community resources and develops resources to meet individual needs and evaluate residents' progress. Supports residents to build a budget and learn personal finance. Completes all required documentation including case notes Enters client data into several databases Participates in all discharge planning, aftercare follow-up, and advocacy Provides Transportation for clients Maintains proper records of expenditures and reconciles them with expenditures account within guidelines and time-frame set by Youth Focus. Oversees educational groups or activities for residents (e.g., desirable health and personal habits, risks encountered by runaway and homeless youth, etc.). Ensures that recreational activities are available to the residents according to program guidelines. May participate or instruct residents in games. Escorts residents to designated activities. Will work on-site and be actively involved with program implementation and delivery of services. Handles all emergency situations according to agency policy and provides timely documentation. Handles on-call responsibilities on assigned weeks Respects the cultural diversity of children in the program, and strives to create a positive relationship. Maintains a safe environment including fire prevention, fire drills and evacuation plans. Maintains any duties necessary to continue the upkeep of the facility. Performs housekeeping duties in staff and client's living area. Uses de-escalation and therapeutic support techniques and procedures with clients exhibiting aggression Gives instruction to clients regarding desirable health and personal habits. Implements behavioral management system and consults with supervisor as needed. Counsels and provides trauma-informed therapeutic services to individuals with mental health issues.

May be assigned other duties as needed by the Program Manager.

Must have the ability to interpret a variety of instructions furnished in written, oral, or schedule form and the ability to draw conclusions.




CONFIDENTIAL DATA




Complies with agency policies and procedures concerning confidentiality and HIPPA regulations involving all aspects of client information, which include day-to-day care of the clients, any case records, family information, medical information, psychological services, etc. which if disclosed, would have adverse internal and/or external implications.
