to join our Skilled Nursing Facility team. The LPN will provide direct patient care, assist with medication administration, support residents with daily needs, and collaborate with RNs and the interdisciplinary care team to ensure the highest quality of care.
Requirements
Experience:
Minimum
1 year of LPN experience
(SNF or long-term care preferred)
License:
Active, unrestricted LPN license in ME
Certifications:
BLS required
Key Responsibilities
Provide direct nursing care to residents in accordance with care plans
Administer medications and treatments safely and accurately
Monitor and document resident conditions, vital signs, and progress
Collaborate with physicians, RNs, CNAs, and other healthcare staff
Educate residents and families on care plans and health management
Ensure compliance with facility policies, state, and federal regulations
Support residents with dignity and compassion in a home-like environment
