Job Description

The Site Case Manager at Flagstaff Shelter Services plays a crucial role in supporting individuals experiencing homelessness in achieving and maintaining housing stability. This position serves as the main point of contact for clients residing at the congregate Huntington Shelter and the non-congregate housing at The Crown and The Lantern. The Site Case Manager will play a crucial role in ensuring effective communication, coordination, and support for clients as they navigate their housing journey. The Site Case Manager works collaboratively with the shelter staff, community partners, and housing agencies to provide compassionate support and advocacy for clients. Responsibilities




Conduct participant intakes, develop individualized service plans, and provide ongoing case management services. Develop and implement individualized client service plans in collaboration with clients, updating quarterly. Meet regularly with clients to facilitate rehousing and promote housing stability. Responsible for outreach and engagement of clients through telephone calls, emails and home visits. Conduct property inspections and report details on client care as required per HUD. Utilize Homeless Management Information System (HMIS) to input and maintain timely, accurate client file and case management documentation. Provides accurate documentation of services, all contacts and information regarding the case including identifying resources, completed referrals, progress notes and detailed summaries of all services and contracts. Track data and provide services in compliance with the CoC HMIS Data Quality Plan and HUD standards. Utilize available financial resources to help move clients forward in their housing journey. Complete monthly progress reports and billing as required. Tracks budgetary spending, makes check and payment requests, and reports all monthly expenditures in compliance with agency and funder policies. Provide reporting to Grants Coordinators, Director of Finance, and Programs Data Manager as requested. Provides non-clinical wrap-around support services with housed clients, including monthly budgeting meetings, updating individual service plans, and accessing other resources as needed. Make referrals to community partners for additional services, as needed. Maintain positive relationships with property managers and community resource partners to facilitate housing and resource opportunities for clients. Provide boundary-driven support to clients and assist them in accessing crisis intervention services and counseling when needed. Collaborate with programs team to develop and facilitate programming for clients. Represent Flagstaff Shelter Services in the community, as needed. Education, Experience, and Qualifications




High school education, GED, or the equivalent Valid driver's license Level I Fingerprint Clearance Card or the ability to obtain a Level I Fingerprint Clearance Card 2 years of experience working in a human services field required or equivalent knowledge, skills, and abilities. General knowledge of Microsoft Office (Word and Excel) Experienced in data entry. Familiarity with HMIS or specialized data entry and reporting databases is a plus.

Environment




Must be able to walk around the property. Perform lifting (25 lbs. or more) Work is performed in a shelter, office, and outdoor environment. Frequent bending, reaching, squatting, and kneeling may be required while performing job functions. Shifts may require long periods of standing and walking. There may be exposure to cleaning agents and allergens. Employees may be subject to loud/frequent noise, disruptive behavior, or human crises involving substance abuse or mental health concerns.
