Position Overview:

We are seeking an outgoing, relationship-driven

Facility Outreach & Marketing Representative

to build strong connections with

social workers, discharge planners, case managers, and administrators

in facilities such as rehabs, nursing homes, and hospitals.



Your goal will be to

introduce our home care services

, demonstrate why we're the

best agency to partner with

, and

generate referrals

for both

private pay

and

long-term care (Medicaid/MCO)

clients.

Responsibilities:

Visit rehab centers, nursing homes, assisted living facilities, and hospitals regularly to establish and maintain relationships. Network with social workers, discharge planners, and other facility contacts to present our home care services. Build trust and demonstrate the quality, reliability, and compassion that set our agency apart. Identify new referral opportunities and convert them into active home care patients. Collaborate with the office team to ensure smooth onboarding of new clients. Represent the agency professionally at community events and networking opportunities.

Qualifications:

Previous experience in healthcare marketing, community outreach, or sales (preferred but not required). Excellent interpersonal and communication skills -- able to build rapport quickly. Self-motivated, reliable, and comfortable working independently in the field. Knowledge of home care, Medicaid MCOs, or healthcare systems is a plus. * Valid driver's license and reliable transportation required.
