New Hire: $23.75 90 days: $24.95 1 year: $26.35 2 years: $27.79 3 years: $29.29



About Ryan White Program Options Counselor:



Provides direct case management and support services to individuals living with HIV/AIDS in Linn County and surrounding areas, ensuring access to medical care, medications, insurance, and other essential services. This position conducts assessments, develops care plans, delivers prevention education, and connects clients to resources while maintaining documentation and collaborating with providers and community partners to improve health outcomes and reduce barriers to care.

Essential Functions

Conducts initial client assessments and develops individualized care plans based on client needs. Coordinates and monitors access to medical care, medications, and insurance coverage. Provides crisis intervention, prevention, and medication adherence education. Makes referrals to community resources such as housing, mental health, and substance use services, and follows up to ensure client satisfaction. Assists clients in determining eligibility and applying for public assistance and insurance programs. Schedules and conducts personal visits with clients in the community and at the office. Completes timely documentation and case notes in compliance with Iowa HHS requirements using appropriate databases. Transports clients to medical and service appointments as needed. Participates in community outreach, events, and agency meetings as a program representative. Maintains required certifications and attends relevant trainings and in-services.

Minimum Qualifications & Requirements

Associate of Arts degree required in social sciences, health or human services. A minimum of one (1) year of experience in hospital, health care, mental health or addiction services is required. This must include community experience serving a high-need, high quantity client caseload of (40+) adults with HIV/AIDS or adults with complex medical, social, mental health and community living needs who require multiple community-based supports and directexperience connecting them to long-term appropriate care or supports. One (1) year of experience determining potential initial and continuing eligibility for public assistance programs required.
