Remote within Eastern North Dakota

- Child Waiver Rural Case Manager




Support families and help create smooth transitions for children on the Autism Spectrum Disorder (ASD) Waiver as a Child Waiver Rural Case Manager with ND Health and Human Services (HHS) in the Medical Services section. This role plays an important part in connecting families with the support they need and requires extensive travel throughout rural communities on the eastern side of North Dakota.


As a

Child Waiver Rural Case Manager

, you will be a key partner for families across the state. You will provide case management services to rural families who have a child with an ASD diagnosis. Your work will include completing intake assessments, developing case plans, and providing ongoing monitoring to ensure families receive the right support and services to improve their quality of life. You will also research available resources within rural communities and develop care plans that help families understand and access the services available to them through the waiver.


You will assist families through major transitions, including helping children move from early intervention services beginning at age three and preparing them to transition to adult services starting at age seventeen. You will also guide families through the process of transitioning off- waiver services and conduct follow-up check-ins one month after discharge to ensure they remain supported.

To be successful in this role

, you must be able to navigate sensitive and sometimes difficult conversations with empathy and professionalism. You will often work with families who are experiencing stress, so the ability to build trust, listen carefully, and communicate clearly is essential. You will also collaborate closely with community partners to ensure families in rural areas have access to the resources and opportunities they need.

To be considered for the Child Waiver Rural Case Manager position

, you must have a bachelor's degree in one of the following areas: social work, psychology, occupational therapy, physical therapy, child development and family science, communication disorders (including audiology or speech pathology), special education, sociology, criminal justice, elementary education, or a related field to the health and human service field AND have two (2) years of professional experience working with children who have autism or related conditions.


A valid ND driver's license is required due to the travel involved in this role.

Location

: ND HHS utilizes a blended workplace model. Depending on the role, employees may work from home, onsite or both. You are required to be in the Eastern part of North Dakota and willing to travel for meetings as needed. You may work from home in this role.
