Job Description


Responsible for leading and supervising designated shifts of skilled nursing and assisted living services in the facility. To provide consistent high-quality care through leadership, supervision and support of the nursing team in identifying resident needs and providing quality clinical care.

:

  • Supervise individuals giving direct care to residents and oversee the implementation of care plans.
  • Supervise the admissions, discharges and transfers of residents as assigned.
  • Participate in developing a program of individualized care of residents. Initiate nursing care plan upon admission.
  • Consult with ADON, DON, Nurse Team Leaders, and physician/NP for special needs of residents.
  • Communicate to other team members, family, and physician of any change in resident condition, and treatment of problem, as is deemed necessary and appropriate
  • Participant in QAPI Committee, as appointed by Administrator and/or Director of Nursing.
  • Monitor compliance with Medicare charting including Hot Charting.
  • Understand and provide supervision for compliance with Federal/State Guidelines and facility policies and procedures for Skilled Nursing and Assisted Living as needed.
  • Responsible for counseling and discipline of nursing staff including participation in 90-day/annual evaluations of nurses and CNAs
  • Use proper safety practices and procedures and correct body mechanics in care for residents.
  • Complete incident reports and investigations as directed.
  • Meet regularly with CNAs and nurses.
  • Acknowledge and resolve customer (residents, families, and staff) concerns.
  • Provide oversight of clinical duties and perform special assignments as directed by Nursing Administration.
  • Responsible for the communication of therapy updates to nursing staff and coordination of education for nursing staff.
  • Leadership and/or involvement in the Electronic Medical Record management team
  • Provide direct nursing care as assigned.
Education & Experience
  • Currently licensed as a Licensed Practical Nurse or Registered Nurse in the state of Indiana required.
  • Minimum of two (2) years experience in a supervisory role.
  • Experience in Long Term Care and/or assisted living. 4. Knowledge of MDS process and care planning
  • Experience with electronic medical records.

  • Knowledge of computers and basic software (email, Word, Excel, etc.) and electronic medical record skills.
  • Leadership, decision making and problem-solving skills.
  • Ability to communicate effectively, verbal and written.
  • Ability to balance multiple objectives in complex situations.
