About the job:

Facility Care Management from discharged stroke patients from Mount Carmel East and Mount Carmel St.Anns.

The Registered Nurse positions are responsible for care in an atmosphere sensitive to each person\'s physical, emotional, social and spiritualneeds within the scope of the Ohio Nurse Practice Act. The Registered Nurse functioning in the spirit of teamwork is accountable for the delivery of patient care utilizing the Nursing Process within the standards, procedures and guidelines of the Organization.

What you will Do:
  • Assists in emergency situations under the direction of the provider .
  • Maintains confidentiality regarding patient and business functions of the center.
  • Administers medications under the direction of the provider, according to the organization\'s standards, policies, procedures, and/or guidelines.
  • Aids in identifying methods of improving patient care.
  • Demonstrates a positive customer service attitude/behavior.
  • Work collaboratively with hospital staff to ensure follow up appointments are scheduled in the appropriate time frame, provide educational materials and resources to decrease readmission rates
  • Facilitates care for stroke population and families for full 30 days after discharge
  • Records data on stroke patients into databases as needed
  • Makes follow-up calls to patients at various time intervals
  • Works closely with the Stroke Coordinators based at the hospitals
  • Maintains current knowledge of national stroke guidelines and educates staff as needed
  • Document and care for patients per the transitional care guidelines
  • Cover for fellow stroke nurses as needed
What we are looking for:
  • Graduate from a credited School of Nursing.
  • Current License to practice as a Registered Nurse in the State of Ohio.
  • One year RN experience required; one-year experience in a medical office preferred.
  • Current BLS/CPR
