Job Description

Under the direction of the Clinic Administrator, the Diabetes Coordinator is responsible for the planning, coordination, implementation, evaluation and management of the diabetes education for the Jackson Clinic. This includes managing human and material resources, program development and promotion, and creating and managing the budget for the department. This position serves as liaison for Diabetes Services to physicians, Administration, the community and hospital staff, and maintains a close relationship with the public relations department for services marketing. This position also is responsible for the planning, coordination, implementation and evaluation of patient and family education throughout the hospital. This includes serving as systems administrator for the Care Notes patient education, computer system, providing staff education related to the provision of patient and family education, and multiple intradisciplinary activities to ensure a quality program. The coordinator demonstrates personal and professional responsibility, participates in creating and atmosphere of mutual trust, acceptance, recognition and respect, and assumes responsibility for own professional development.



Requirements:



Graduate of an approved professional nursing program. Masters degree in nursing or education required. Four to five years as a Registered Nurse in a management position in a clinical setting; two years experience in adult education and program planning; experience working and communicating with all levels of health care workers, physicians and administrative personnel. Experience with a wide range of cultural, age and educational diversities within a patient population. Computer skills with Microsoft products. Current licensure as a registered nurse in the State of Alabama. Must have CDE certification or be eligible to take CDE exam; must pass the CDE exam within two successive attempts. Able to communicate effectively verbally and in writing; able to analyze situations, recognize problems, search out pertinent facts, and reach sound conclusions; demonstrated competency in the organization and presentation of instructional materials; knowledge of principles of education for multiple age groups. Ability to effectively chair and serve as a contributing member of intradisciplinary committees. Excellent time management and critical thinking skills. Ability to form effective working relationships with employees, management, physicians and patients. Hearing and visual acuity. Able to lift and handle supplies and audiovisual equipment. Able to communicate verbally and in writing. Able to meet the physical demands of the job with assistive or adaptive devices or reasonable accommodation. Able to meet standards of health established by law, regulation or policy. Ability to assist in lifting, pushing and pulling patients/equipment involving a minimum of 50 pounds. Ability to meet the attendance requirements of the job.
