Suffolk County Nashua Street Jail in Boston, MA
Summary:
Responsible for supervising the delivery of Medication for Opioid Use Disorder (MOUD)
provided at the correctional facility on a daily basis.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
The MOUD Care Manager serves as the primary nursing support for medical providers delivering Medication for Opioid Use Disorder services. The MOUD Care Manager provides education and support to patients with chronic substance use conditions and, in partnership with the patient and medical staff, develops a care plan that is the right fit for the patient's condition(s) to ensure needed access to behavioral health and medical services and education/supports self-management of medical condition(s).
? Provides appropriate follow up on patient referrals, ensures orders are updated correctly
? Participates in and/or facilitates internal and external case conferences, as well as other
agency or community-based meetings or trainings as needed
? Maintains current case records and other documentation as required and prepares
necessary reports on time, checks log books regularly
? Demonstrates knowledge of stages of change, substance use disorders, services, and
family dynamics as well as common related medical and mental issues
? Completes on-going training to enhance skills and patient care
? Orders medication and other medical supplies needed for the program
? Conducts quality improvement projects and quarterly chart review to evaluate and
improve the quality of services
? It is an essential part of the job that CPS can ask the employee to work beyond his/her
regularly scheduled hours
? Prepare for program audits to include but not limited to the Department of Public Health
(DPH) and Drug Enforcement Agency (DEA)
? Creates and maintains dosing nurse schedule and covers shifts as needed
? Provides supervision to all dosing nurses
? Creates and maintains provider(s) schedule(s)
? Sends in bridge prescriptions for releasing patients as needed
? Follow all security rules and regulations
? Perform other duties and responsibilities as assigned.
MINIMUM REQUIREMENTS:
? Graduated from an accredited nursing school
? 2-5 years MOUD/OTP experience (required)
? 2-5 years MOUD manager experience (preferred)
? Maintains a current nursing license
? Maintain current CPR certification.
