Mercer Home Health Care is seeking a per diem registered nurse- liaison needed to maintain continuity of care and transitioning of the patient from acute care facilities to home care. The candidate will establish and maintain good working relationships with acute facilities/ discharge planners /social workers. The candidate must possess strong interpersonal skills needed for professional relationship development, knowledge of skilled nursing, hospital discharge and transitional care. The candidate will gather and prepare information in advance of patient transfers, track data related to transfers and compile information into reports .
The candidate will fill in for coverage when needed at our acute care facilities and skilled nursing facilities.
