As a PeaceHealth Home Health RN, you'll provide one-on-one skilled nursing care to patients recovering or managing chronic conditions in their home environment. You'll assess, plan, coordinate, and evaluate patient care while empowering patients and families through education and support. Every visit you make helps promote healing, independence, and peace of mind.

What You'll Do

Conduct comprehensive patient assessments and determine eligibility for home health services. Develop and manage individualized care plans in collaboration with physicians and the interdisciplinary team. Deliver high-quality, compassionate nursing care and provide instruction to patients and caregivers. Coordinate care with Home Health Aides and other clinical partners to ensure seamless delivery of services. Document all care accurately and promptly in compliance with agency and regulatory standards. Support families and caregivers with education and bereavement resources when needed.

What You Bring

Graduation from an accredited school of nursing required;

BSN preferred

.

Two years

of acute care or home health/hospice nursing experience required. Current

RN license

in Oregon,

BLS certification

,

driver's license

, and proof of auto insurance required. A passion for delivering patient-centered care and the ability to work independently in a home setting.
