Job Summary

We are seeking a compassionate and highly skilled Registered Nurse to provide exceptional home health care services. This role offers the opportunity to work in a dynamic environment, promoting patient recovery and well-being within the comfort of their homes. The Registered Nurse will be responsible for coordinating care, educating patients and families, and ensuring compliance with healthcare regulations such as HIPAA.

Duties

Manage patient care plans from the office, coordinating services for home health patients. Work with physicians, patients and caregivers to ensure care plans are implemented effectively. Perform clinical assessments, review patient information and communicate necessary updates with the home health team. Oversee the coordination of medical equipment, supplies etc. for home health patients. Monitor patient progress and make adjustments to care plans based on patient needs. Provide clinical guidance and support to home health staff and caregivers. Ensure proper utilization of resources to promote effective and efficient care.

Qualifications

Valid Registered Nurse (RN) license in Hawaii. 1-2 years of experience in home health care, case management, or office-based healthcare coordination. Experience in hospital medicine, post-acute care, or outpatient clinics is highly desirable. Knowledge of medical terminology, physiology, and diagnostic evaluation techniques. Proven ability to perform comprehensive patient assessments and discharge planning. Familiarity with HIPAA regulations and medical records management. Strong skills in patient service, case management, and utilization management. Excellent communication skills for effective patient education and interdisciplinary collaboration. Ability to work independently and collaborate with multidisciplinary teams.
