Responsibilities

Monitor patient's condition and assess their needs to provide the best possible care and advice Observe and interpret patient's symptoms and communicate them to physicians Collaborate with physicians, various disciplines, and the patient to devise individualized care plans Perform routine procedures (blood pressure measurements, administering injections etc.) and document in patients' charts Adjust and administer patient's medication and provide treatments according to physician's orders See patient's in their homes or senior living communities Provide instant medical care in emergencies; AHA BLS for Healthcare Provider required. Foster a supportive and compassionate environment to care for patients and their families Participate in case management review and care conference meetings Participate in weekend rotation and minimal call

Proven experience as a registered nurse Enjoy problem solving and educating people A team player with excellent communication and interpersonal skills Responsible and compassionate Outstanding organizational and multi-tasking skills Oasis competent 1 year Home Health Case Management experience
Job Types: Full-time, Contract

Pay: $60.00 - $85.00 per hour

Expected hours: 20 - 40 per week
