Deliver skilled, compassionate nursing care according to physician orders and patient-specific plans Conduct in-home assessments and create individualized care goals Serve as a

, collaborating with interdisciplinary team members and supervising field staff Provide health education and guidance to patients and their families Document care accurately and efficiently using our EMR system Maintain accurate clinical records in compliance with state and federal regulations

Qualifications

Active

RN license

in the state of Indiana At least 1 year of professional nursing experience (

home health experience is a plus

) Strong critical thinking, organization, and time management skills Ability to work independently in the field Reliable transportation, valid driver's license, and auto insurance
