to oversee and coordinate patient care services. This role ensures high-quality care delivery, provides clinical oversight to staff, and supports an interdisciplinary approach within our hospice/home care programs.
The ideal candidate is a strong leader, skilled clinician, and excellent communicator with a deep understanding of hospice philosophy, patient-centered care, and clinical best practices.
Key Responsibilities
Receive and review case referrals, including home visit assessments, to determine patient care needs.
Assign appropriate hospice/home care personnel and collaborate with the Medical Director regarding patient eligibility.
Evaluate each case through home visits, conferences, and documentation review to ensure quality care.
Guide and support clinicians to promote effective performance and best practices.
Review patient diagnoses, medications, and clinical progress.
Assist clinicians in setting goals, prioritizing care, and developing individualized care plans.
Participate in case conferences to ensure coordinated, interdisciplinary care.
Conduct quarterly record reviews and communicate findings to the Clinical Director.
Assist with screening, interviewing, onboarding, and orienting new hires.
Support planning and evaluation of in-service trainings and continuing education programs.
Contribute to developing and updating organizational policies, procedures, and goals.
Participate in community outreach and promote the organization's mission and values.
Engage in quality assessment and performance improvement activities.
Perform other related duties as assigned by the Clinical Director/Director of Patient Care Services.
Qualifications
Current, active RN license in the state.
Graduate of an NLN-accredited nursing program;
BSN preferred
.
Minimum of
1 year of nursing experience
within the last 3 years.
At least
2 years of supervisory or management experience
in a hospice or home care setting.
Knowledge of hospice philosophy, end-of-life care, pain/symptom management, and interdisciplinary care.
Strong clinical judgment, communication skills, and leadership abilities.
Must maintain current CPR certification.
Must have a valid driver's license and a reliable, insured vehicle.
Ability to adhere to professional standards and regulatory requirements.
