Job Description

The Hospice RN Case Manager is responsible for coordinating all aspects of care for patients and families receiving hospice services. This role ensures high-quality, compassionate, patient-centered end-of-life care through comprehensive assessment, care planning, education, and ongoing clinical oversight. The RN Case Manager serves as the primary point of contact for patients, families, interdisciplinary team (IDT) members, and physicians.

Job Type: Full-time

Pay: $85,000.00 - $95,000.00 per year
