Vitality at Home is seeking RN, Case Managers in St. Cloud, MN. Successful candidates will ensure the health and safety of home care patients through comprehensive assessments, individualized care planning, ongoing monitoring and staff supervision. This role supports each patient in achieving their highest level of independence while ensuring compliance with state and federal regulations.

Required Skills/Abilities:

Strong knowledge of adult clinical care and home care regulations Proficient with Windows-based systems and healthcare software Effective planning, time management, and ability to work independently Excellent written and verbal communication skills. Must understand English proficiently. Professional demeanor with flexibility, integrity, and confidentiality Commitment to service excellence and a collaborative work environment Regular interaction with patients, staff, and community members Must be able to maintain concentration, manage multiple tasks, and respond quickly to changing conditions Strong attention to detail and memory recall Ability to manage multiple tasks, prioritize, and problem-solve effectively Requires reading, writing, basic math, and the ability to communicate clearly Comfortable working independently or in physically isolated settings when needed Capable of handling patient and staff concerns with professionalism

ducation and Experience:

Current licensure as a Registered Nurse RN in the State of Minnesota.
