ABLE, Inc. is seeking a dedicated Registered Nurse Case Manager to provide comprehensive health care services to individuals with developmental disabilities. This role involves case management, health supports, staff training, and ensuring the well-being of individuals in our care.

Key Responsibilities:

Case Management:

Conduct comprehensive assessments and develop individualized care plans.

+ ####

Coordinate services and supports to address unique health needs.

+ ####

Monitor progress and revise plans as needed.

+ ####

Facilitate opportunities for growth and independence.

+ ####

Manage financial resources and coordinate with external agencies.

+ ####

Ensure compliance with applicable laws, regulations, and policies.

####

Health Supports:

+ ####

Monitor physical and mental health needs, including medication management and appointments.

+ ####

Ensure adequate environmental support and assistive devices.

+ ####

Provide training and instructions to staff.

+ ####

Assist with hospital admissions, discharges, and transportation.

####

Staff Training and Development:

+ ####

Provide training to direct care staff on nursing tasks.

+ ####

Ensure competency of staff in delegated nursing tasks.

####

Qualifications:

####

Bachelor's Degree and Registered Nurse license in North Dakota

####

Valid driver's license

####

Proficiency in computer systems (Outlook, Teams, Therap)

####

Experience with ICF/DD and The Council standards preferred

####
