$60 hr. ($2,400 per week)

Description:

We are seeking a

RN Case Manager

for a home health program. This temp-to-perm role involves providing skilled nursing care, managing patient care plans, coordinating with healthcare providers, educating patients and families, completing home visits, and documenting care in the EMR.

Requirements:

Current RN license in Montana BLS/CPR certification Minimum 2 years of nursing experience preferred Experience in case management, home health, or patient education Reliable vehicle for local travel Ability to work independently and collaboratively
