:

The Community Care RN provides and supervises professional, independent nursing care for patients. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. Applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients\' optimal health and independence.

Required Qualifications:

• Oregon Registered Nurse License upon hire.
• National Provider BLS - American Heart Association upon hire.
• 2 years - RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience.

Preferred Qualifications:

• Current recognition by a national professional organization in care management, geriatrics or hospice upon hire.
• Bachelor\'s Degree - Nursing.
