We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality patient care in a home setting, ensuring the comfort and well-being of patients while promoting their independence. This role requires a strong clinical background, excellent communication skills, and the ability to work collaboratively with patients, families, and interdisciplinary teams.
Responsibilities
Conduct comprehensive patient assessments and develop individualized care plans.
Administer medications and treatments as prescribed, including IV infusions and injections.
Monitor patient vital signs and report any changes in condition to the healthcare team.
Provide education to patients and families regarding health management, medication administration, and disease prevention.
Coordinate care with other healthcare providers, including physicians, therapists, and social workers.
Document all patient interactions accurately in electronic health records (EHR) systems.
Ensure compliance with HIPAA regulations and maintain patient confidentiality.
Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming as needed.
Perform wound care, catheterization, and other specialized nursing procedures.
Participate in case management and discharge planning for patients transitioning to home care.
Skills
The ideal candidate should possess a variety of skills including but not limited to:
Proficiency in EMR systems such as Epic or Cerner.
Experience in critical care settings or emergency medicine.
Knowledge of ICD-9 and ICD-10 coding for accurate medical documentation.
Familiarity with hospice & palliative medicine practices.
Ability to perform acute pain management techniques.
Experience working with individuals with developmental disabilities or memory care needs.
Competence in performing physical examinations and diagnostic evaluations.
Strong understanding of infection control practices and aseptic techniques.
Ability to provide geriatric care including managing chronic conditions.
Experience with home care services or long-term care facilities.
