We are looking for a

Rural Health Transformation (RHT) Contract Monitor

who will be responsible for the management and monitorship of assigned contracts and subrecipient deliverables related to the RHT Program. This position supports the RHT Program Director with managing the yearly grant cycle, including execution of new contracts as well as amendments and renewals of existing contracts. The Contract Monitor works closely with the RHT Program Coordinators and Fiscal Analyst to ensure RHT program compliance. The RHT program is a federal initiative led by CMS and focuses on promoting innovation, strategic partnerships, infrastructure development, and workforce investment to help state governments strengthen healthcare access, quality, and outcomes in rural communities. This position reports directly to the RHT Program Director within Health Programs.Provides day-to-day oversight, follow-up, and maintenance on contract management and subrecipient deliverables for assigned contracts. Tracks, assists, and escalates as necessary the planning, coordination, administration and execution of new contracts, contract renewals, and contract amendments. Establishes contract monitoring cadences in collaboration with RHT Program Team. Measures, tracks and reports regularly on contract activity, contractor performance and agency satisfaction to RHT Program Director and others. Archives and stores supporting documentation, communications, reports, etc. Works with the RHT Program Coordinators to provide input and prepare communications for publication to subrecipients, internal and external stakeholders. Assists in the development and maintenance of all procedural documentation, tools, and reports, etc. essential to properly monitor, analyze, measure and report contract and subrecipient deliverables and compliance. Evaluates, updates, and maintains all resources to ensure accuracy. Maintains contract monitoring protocols, practices and plans tailored for each assigned contract, including the collection and storage of data required to monitor, analyze, and report compliance. Documentation is accurate, concise, and easily followed and utilized by others. Supports the RHT Program Director with the annual grant cycle and review/evaluation process. Ensures grant cycle and review and evaluation processes remain on track within established timelines/deadlines. Serves as grant application reviewer. Provides reports to RHT Program Director and other stakeholders as necessary on status of grant cycle and review process. Archives and stores supporting documentation, communications, reports, etc. Continuously improves annual cycle based on lessons learned. Measures, tracks, and reports regularly on status and escalations for assigned contracts. Provides day-to day oversight, follow-up, and maintenance of assigned contract/subrecipient related escalations. Archives and stores supporting documentation, communications, reports, etc. Develops and submits after action review documents to address repetitive escalation topics to management. Leads or supports other projects proactively or as assigned by Agency leadership. Continuously engages in process improvement to support Agency and Division-wide strategies, objectives, and initiatives. Occasional travel may be required as assigned.

Minimum and Additional Requirements

A Bachelor's degree in a healthcare or business related field and experience in business, personnel, grant-in-aid, public administration, or related field.



An equivalent combination of experience and education may be considered with prior State Human Resources approval.



Additional Requirements:





Overtime and/or weekend work with Deputy approval. Sitting or standing for long periods of time. Lifting requirements: 20 lbs. Valid driver's license. Occasional overnight travel.

Preferred Qualifications

Three years (3) of work-related experience. Demonstrated competencies related to contract procedures, functions, and compliance; Competent in use of protocols, policies, procedures, and tools; Ability to work independently; to organize and complete work assignments with minimal guidance; Ability to maintain a positive working environment with internal and external relationships; Capable of following oral and written policies/regulations; Proficient in serving as point person for day-to-day oversight of assigned programs; Working knowledge of financial and accounting practices. Familiarity with state or federal health programs, Medicaid, or community-based healthcare delivery preferred.
