Job Description

1. Participate in IDT Care Planning process.
2. Perform all restorative services for residents based on their individualized plan of care.
3. Work in direct correlation with the licensed nurses and Rehabilitation Therapists when
indicated in providing restorative services.
4. Notify the charge nurse immediately of any significant change in patient's mental or
physical condition.
5. Accurately record the residents participating in the program in POC (Point of Care) in
PCC (Point Click Care) the electronic health record.
6. Work with the Staff Development Director with in-services that are relevant to
Restorative Nursing. Perform all Restorative Nursing Programs in which he/she has
been instructed
